Incompetent. Had license previously suspended (see IDPR website). Sued for malpractice and other issues multiple times (ex. failure to provide refund when work not done). Does not disclose that manufactures implants himself. Implants almost always fail. Promises a # of implants but always ends up putting less because puts one implant where there is room for two. Doesn't know what he's doing despite years of practice. Stay away.

Two implants and both fell out. He took my money from the Carecredit account before he did any work. This guy is a bad dentist as well as a liar and cheat.

About Front Desk: Also lies to cover up for this bogus dentist.

I had 2 implants installed by Dr. D'Alise when he had offices in downtown of Chicago and in Oakbrook. I don't trust this dentist anymore because of the following: First, a year or so after he put the crowns over the implants, these crowns fell off. I found that the crowns were defective: both (porcelain?) crown had holes in their adjacent (inner) sides. The holes were covered with some kind of yellow plastic or glue(!) - to hide them from a view. What is very unpleasant is that he knew about the holes when he had them installed over the abutments but hid it from me. Though he replaced the crowns, per my request, it took long time to catch him and make him to re-do this (at this time, he had an office in Glenview, IL, close to my location). Then, in another year or so, one of the crowns he re-installed, fell off and was lost. When I came to him (again!) he made me to pay ahead (not a full price, though) for a new crown, promising it will be ready in two weeks. He did not offer me a temporary crown and did not offer a choice of a shade for this future crown. On the top, in two weeks the crown was not ready, and his office did not know when it will be. D'Alise was not reachable for questions.

About Front Desk: Not too polite.

About Hygienist: 1 star is too good, but I give it to him, just because the implants have not fallen off yet, as his crowns have.

Having him do two implants was the biggest mistake of my life. He found a way to get my money before he did all the work and then did a horrible job. The work will have to be done all over again at even more expense because the mess he created has to be redone. After having all my problems, I then looked up his record with the State of Illinois Dept. of Professional Regulation and found he had been suspended and also disciplined by them. He also bankrupted his practice during my treatment period and then disappeared. I finally found him but he wouldn't return my calls until I got my State Representative after him. I am stuck with major expense now but plan on seeking legal recourse. BEWARE!!!!

Like most people, I decided to do implants with A Center for Dental Implants because of the very attractive price point. But every time I do something stupid like this, I am reminded of the old adage: "You get what you pay for." By way of background, I have done a few implants with this doctor and have not been satisfied with the quality of his work. His implants don't hold up and his skill is abysmal! Additionally, the post he uses to attach the crowns are archaic and do not bond very well. I am currently in the process of getting some of my implants redone. Sigh! I have sought out second opinions from very competent implant surgeons and they were shocked to see the poor quality of his work! On another note, there is constant turnover of staff which is a very bad indicator of the state of affairs at this place. Bottom line: Do your research and don't make your decision on price alone!
