Crown fall off after 3 years of placing it, total waste of money

I have following Dr Mesa practice since my youngest daughter was 6 months now 9 1/2. He has been very patient and compliant with me and my family. His staff always calls to remind me it's time for a check up, which I find great because otherwise I forget.
I've and my family have has numerous procedures such as root canals, impacted wisdom tooth, fillings crown, and regular check ups. I've also throughout the years have had various insurence and he always seem to work with it.

I started seeing Dr. J Mesa because i had pain, i got an appointment the same day, he did a root canal and my pain was gone the next day. at a latter appointment he did the crown and I am very satisfied with his service. I am refering my family and friends to him.

Very poor communication skills plus minimum of a 45 minute wait after scheduled appt. time then you end up waiting in the exam room as he jumps between 2-3 patients at a time. He left me in the middle of a root canal for over 30 minutes that the anesthia wore off yet he continued to attempt to proceed with the procedure. It took 4 visits before I gave up on him on getting the job done and went to another dentist. And with an infection due to a poorly handled 3rd attempt on the root canal.
