Dr. Berenstein is advertized as a Preferred Provider for Delta Dental Insurance Company. As such, he contracts with Delta Dental to accept the reimbursement rates set by Delta Dental for periodontal procedures. On the morning of surgery, 4/26/2010, his office manager demanded full payment up front, $2130. Delta Dental paid him their set fee and told him my share should have been only $527. Over 5 months later, I'm still writing letters to Delta Dental because he still owes me $520.

I have aetna and this Dr is an in network provider. He still said he won't do a pre certification and to pay cash up front. I was in so much pain I had no choice. My insurance said it payed 100% of the bone graft and extracttion. This office even after my insurance company paid him in full refused to reimburse me. It is going on 6 months and I still haven't received my money for the fees I payed up front. The Dr called me a baby and the office manager Nancy hung up on me and my insurance company several times.
