I've been with Dr. Koike ever since a previous dentist messed up a root canal. My wife has been with him for something like 3 decades.
Dr. Koike himself is friendly and soft spoken. He or any member of his staff will take the time to answer questions, as well as provide solid advice on the upkeep of dental health.
I listed the cost as a "3", but my wife handles the payments and insurance, so I'm not really qualified to answer that question. However, I decided to lean toward a favorable number.
I actually live in Mililani, but the quality of care and professionalism makes the trip well worth it. On a more "unbelievable" note, there are actually current periodicals in the waiting room!
About Hygienist: Dr. Koike's daughter performs as my hygienist. She has always done an excellent job every time I've gone for a cleaning. Quick, very thorough, polite, and professional. She's good enough to know if I'm favoring one side too much when I brush, as well as accurately determine how much (or how little) I've been flossing.
