Dr. Karnofsky did me a favor when I needed a periodontal maintenance cleaning because I had dust in my gums. The hygienist who did it was very nice, and she has a great baking soda cleaning which removed the stain from my hard-to-clean teeth, which many dentists don't want to clean, because I smoke.
However, I had one tooth which had been filled many times, top and bottom, and in doing the exam, the doctor accidentally dislodged a piece of the filling. The top of the tooth had been refilled with a white filling. So at home, I tried to clean off the smoker's stain with a scaler and removed two fillings by accident, including the one the doctor had dislodged. Then later I ate some candy and the tooth broke in half. I went to an oral surgeon who extracted the tooth. Probably any one of these many dentists would be happy to give me an implant, but I figure who knows what else will go wrong. I'll just leave the space there. I eat now on the other side, and have lost weight, but I am happy.
About Hygienist: She is very nice, gentle, thorough, and painless. Highly recommended.
