I went to a dentist to get a general cleaning and deep cleaning back in 2013 from Ricardo Castaneda, DMD. My insurance through Humana and it sucked, so I had to pay most of the cost out of pocket since my insurance company didn't pay for much. I went to the doctor 2 times in a few weeks and also had to pay 2 times for the two services. One day was a general cleaning and the other was a deep cleaning.
In 2015, I got a letter in the mail from my previous health insurance provider (aka Humana) that they have received another dental insurance claim from this doctor for my visit back in 2013. I found this awkward since I had already paid everything that was asked of me more than 2 years prior to getting this letter from the insurance company. Just for a note, this letter from the insurance company said this was not a bill, it was rather a letter letting me know that another claim was trying to be made from a medical provider.
I just logged into my credit report on August 19, 2016 and saw that my credit report had taken a huge plunge and saw that it was due to the collections notice reported to my report from First Federal Credit Control for unpaid debt to Ricardo Castaneda, DMD. This was very confusing and upsetting to me since I was never contacted by Ricardo Castaneda about this bill once, I was never contacted by First Federal Credit Control about this bill by phone nor by mail once.
I find this very upsetting since collections agencies have to send you a notice letting you know you have an outstanding balance with them with a medical bill - this was never done.
As I said, I was never contacted by Ricardo Castaneda, DMD about this bill once, nor was I ever contacted by First Federal Credit Control by mail or by phone about this bill once. They just reported it to my credit report more than 3 years after my visit.
I believe this is health insurance or medical fraud because I paid everything I was asked to pay at the dental office and nothing else remained. However, more than 2 years after my visit, the dental office tried to file a new claim against me for to Humana for my visit after 2 years ha past.
I am reporting this claim to the BBB first in order to give First Federal Credit Control the opportunity to fix it before I file a complaint with the Federal Trade Commission and a lawsuit.
I'm also going to be filing a report with the dentist and his office as well for unfair and illegal healthcare and medial fraud practices. I'm also going to be filing a report with Humana as well to let them know what the dentist as well as First Federal Credit Control are doing.
OUTCOME
My desired outcome is to get the negative collection mark removed from my credit report. If this happens I will be very happy. I will still be very upset by happy. I still do not understand how you would go visit a doctor, pay for your services at the doctor's office and everything is said to be paid in full, then am another insurance claim is filed with the same insurance provider more and 2 years after you visited the doctor's office. Then having a medical debt reported as collections negative mark on your credit report without being notified by the medical provider or collections agency ever (not even once) in writing or by phone that you have an outstanding balance.
To me this just screams medical and/or health or dental insurance fraud.
