At your first appointment you fill out a form stating what you would like to happen regarding the current treatment and list any future desires you might have regarding your dental health. Dr. Hayhurst consults this form in providing a plan for first improving your dental health, and second for giving you cosmetic dental options. If you want a proactive dentist I would recommend Dr. Hayhurst. (Treatment: root canals, crowns, bridge, cavities, teeth cleaning, invisalign).
