I had a crown installed NOV 2013. It fell out DEC 2014.
Dr Jacobson appears to be 30 years old. How could the previous reviewer have been going to him for 25 years?
About Front Desk: I found his staff to be lacking in professionalism.
About Hygienist: More interested in talking with co-workers than explaining procedure to patient and did not relay patient conversation to Dr correctly.
