On December 10, 2015, during oral surgery to extract my wisdom teeth, Dr. Jeffrey Kootman left half of my #32 tooth inside the socket. I returned a week later to have my sutures removed and found that Dr. Kootman forgot we had an appointment. An hour after our regularly scheduled appointment, Dr. Kootman returned and misdiagnosed his failed surgery as complete.
In February of 2016 after weeks of pain and doubt that the surgery had been successful, I visited a different dentist while on a business trip in West Palm Beach, Florida. There I learned that the #32 tooth remained in the socket and had an X-Ray taken as proof. At that time I chose to have emergency surgery to alleviate the pain. I had the tooth extracted and paid the additional dental fees out of pocket. I returned to Gentle Dental in February of 2016 immediately after the emergency surgery and requested that I be refunded for this mistake.
After not receiving any payment or communication after my visit to Gentle Dental in February, I returned in April only to have to start the entire process over again. THIS GUY IS A QUACK!!

I had two teeth removed with sleep. It went very well and the doctor and staff were very friendly. The office is clean and nice. I would recommend the office for your oral surgery needs.

I went to this Dr for a sever jaw problem and after 5 mins and him berely looking at me he said "there is nothing wrong with you." and walked out. Really could he not see that my RT side of my face was swollen? Could he not see that I could not open my mouth? Oh wait I forgot you have to actually examine someone to see that!! Well Dr Kootman, I have been seeing a real Dr for the past 6yrs and I have to tell you I did have something wrong with me. Umm had to have a complete RT Jaw replacement. So glad it was not you who did it! Whatever you got out of my insurace copanies was way way too much!

Staff was friendly. Procedure went as expected. I had a wisdom tooth extracted by Dr. Kootman.

Dr. Kootman Did emergency extractions for both my father and I. We couldn't afford to pay anyone else. I went in with an abcessed tooth and Dr. Kootman worked with my finances, and had a great bedside manner. Two years later my father had a horrible problem, and once again Dr. Kootman worked with us, and even did the extraction that day. I reccomend him to anyone who needs oral procedures done.

The office insisted their fees would be a certain amount and we would owe them over $700. The insurance company processed the claim and our financial responsibility was over $500 less than what we paid. Dr Kootman's office is now saying they will only refund us $300. It is not a time in our life, nor anyone elses who we know where we can afford to be short $500.

I had two implants placed into my lower jaw. These held my lower denture in with great success. Very caring and called me that night to check on me. I would recommend Dr. Kootman to all my friends and family as he and his staff are very caring!!!

My son, who has a terminal disease, was referred to Dr. Kootman for removal of a tooth. My son had been in terrible pain for about three weeks. When I called for the appointment I told them he had Delta Dental, which covers 100%. I brought a copy of his xray from the referring dentist and the referal paper. Before they would work on my son I had to pay $192, which the office receptionist said was a 20/80 payment from insurance. I knew this was not true but felt I had no choice due to pain my son was in. This was a simple extraction with no medication other than one shot. While they worked on son I called Delta and they spoke directly to Kootman's office gal and told her it was covered 100%. Still Kootmans gal refused to refund my money and told me I would have to wait six weeks for a refund. I told her my son has very little money as he is on disability but she did not care and told me he would get a refund in six weeks. If I paid 20 %, that would mean he is billing the insurance over $1,000 for a simple extraction. Something very wrong with this. His office was full of other patients on ACCHS and possibly Delta. Two times Delta tried to fax the benfits to Kootman but the office gal gave them the wrong fax number TWICE.

I was overcharged for services but only knew this after I called my insurance to ask about why the cost was so much. Dr. Kootman' s office confirmed that I was due a credit back for almost $300. I keep calling their office and no one ever returns my calls. The surgery went well but I would really like my refund on what is owed to me.
