July 13, 2013 I was in this dentist office and the following services she has performed on me:
1) Prophy - adult
2) comp. oral eval
3) FM xray incl BW
4) blood pressure
5) Composite - surf
This doctor office claimed with my insurance for $1,795.00.
I called this office and did not get clear explanation.
For service 5), this office claimed for 7 times and apparently the doctor did only one location of my mounth.
